MAMA’s streamlined process is patient-friendly and our clinic is comfortable. We specialize in helping patients understand the Program registration process, get needed medical records and schedule appointments with doctors knowledgeable about cannabis therapeutics.
Our criteria for acceptable medical records are in accordance with state regulations and are as follows: records must be recent i.e. within the past year in most cases, and from an M.D. or D. O. (Alternative treatment must be documented and show on-going treatment). Please limit this to a few pages. You can bring other pertinent medical records to the clinic at the time of your scheduled appointment, however we must receive your medical records prior to setting an appointment.
MAMA's Appointment Process -
- Your first step is to instruct your current physician to send your medical records to the MAMA office where you want to attend a clinic. To do this, you must send a Medical Records Release Form to your current physician.

- Print it and complete it with your most recent physician's information.
- Initial the boxes, don't just check them. Remember to sign and date the form.
- When the form is completed and signed, send it to the records department for the physician from whom you are seeking medical records.
- BE PATIENT! It can take up to 30 days for your doctor to send your records to us.
- When we have received your records, we will call you to schedule an appointment. Until we have received your records, we cannot go forward. If you have not heard from us three weeks after sending your doctor a Medical Records Release Form, contact us and we'll do what we can to expedite the process.
