MAMA's Medical Marijuana Clinics

MAMA has 3 clinic locations in which to better provide you with a wide variety of services including classes, socials, books and great products.

New patients are $200.00 and Returning MAMA patients are $180.00.

As a nonprofit, we try to keep our patients’ prices as low as possible, but we also have to meet our financial obligations. Our clinic fees directly fund our clinic services with no room to spare. Our dedication to excellence and education is unwavering and we will continue to provide this level of service at all of our clinic appointments. 

How do I start the Process?

Send current medical records to MAMA for review. You can fax them to 1-866-559-3369, or mail them to

Please make sure to include your complete name, date of birth, and contact information.  Let us know the best ways to reach you for scheduling your appointment.

Records take 5-7 business days to be reviewed once we have received them from you or your provider.  If you have not heard from us within 1 week of us receiving your medical records, please call us at 541-298-4202 for further information. 

We will soon contact you to verify your records and get you scheduled at the MAMA office you prefer, whether in Portland, The Dalles or Bend.

MAMA's Appointment Process

MAMA’s streamlined process is patient-friendly and our clinic is comfortable. We specialize in helping patients understand the Program registration process, get needed medical records and schedule appointments with doctors knowledgeable about cannabis therapeutics.

Our criteria for acceptable medical records are in accordance with state regulations and are as follows: records must be recent i.e. within the past 12 months in most cases. Records can be accepted from an M.D., D. O., Nurse Practitioner (N.P.), or Physicians Assistant (P.A.). For return patients: Alternative treatment must be documented and show ongoing treatment. Please limit this to a few pages. You can bring other pertinent medical records to the clinic at the time of your scheduled appointment, however we must receive your medical records prior to setting an appointment.

  1. Your first step is to instruct your current physician to send your medical records to the MAMA office where you want to attend a clinic. To do this, you must send a Medical Records Release Form to your current physician. Click here to download our MAMA's Medical Records Release Form. 
  2. Print the records release and complete it with your most recent physician's information.
  3. Initial the boxes, don't just check them. Remember to sign and date the form.
  4. When the form is completed and signed, send it to the physician from whom you are seeking medical records.
  5. BE PATIENT! In most cases, records come sooner, but it can take up to 30 days for your doctor to send your records to us.
  6. When we have received and reviewed your records, we will call you to schedule an appointment. Until we have received your records, we cannot go forward. If you have not heard from us two weeks after sending your doctor a Medical Records Release Form, contact us and we'll do what we can to expedite the process.
